by Dr. Earl MindellLess HRT = Less Breast Cancer?
In the aftermath of the Women’s Health Initiative, the study halted in July of 2002 because of an unacceptable increase in the risk of breast cancer in women who were taking Premarin plus a progestin; some 50 percent of women on HRT (hormone replacement therapy) quit cold turkey. It was probably the biggest health news story of that year.
Millions of women had been taking these drugs, trusting that not only were they controlling troublesome symptoms of menopause, but that they were also protecting their cardiovascular systems and their bones. They were right about the bone-protective effect, and they were protected against colon and ovarian cancer too—but they also were found to be at increased risk of heart attacks and strokes with this HRT regimen.
Even now, some three and a half years later, this is a big topic of conversation among physicians, health journalists, and women who are entering menopause. Some physicians continue to prescribe Premarin/progestin HRT to women who direly need help with menopausal symptoms. But in December of 2006, a study came out that seemed to put the final nail in the HRT coffin: the study revealed that breast cancer diagnoses had declined by 7 percent in all age groups, and by 14 percent for the type of breast tumor that is promoted by estrogen. (The rate had previously climbed by about 1.7 percent per year from 1990 to 1998; it then climbed more slowly, at 1 percent per year, from 1998 to 2002.) The study was led by Donald Berry, Ph.D., chief of biostatistics at the University of Texas’ M. D. Anderson Cancer Center in Houston, Texas, and was presented at the annual SABCS (San Antonio Breast Cancer Symposium) on December 14.
The headlines shouted that it was the drop in HRT use that made the difference. Although I’m no fan of Premarin and progestin HRT, I find myself a bit skeptical on this front. The data in the study cover only a year’s time, finding that there were about 14,000 fewer breast cancer diagnoses in 2003 than in 2002. That’s a lot of women saved from a terrifying diagnosis, no doubt—but breast cancer is a disease that takes longer than weeks or months to develop. Many experts think that the withdrawal of the tumor-accelerating effect of HRT may be the reason for this shift; in other words, the cancers are still forming in women’s bodies, but they are growing more slowly and can’t be detected as soon.
The drop in diagnoses is probably due in part to less use of pharmaceutical HRT, but there are probably a lot of other factors in play. Maybe women are eating better, exercising more, losing excess weight, and taking better care of themselves. Maybe there are important differences between women who are entering high-risk age ranges now and those who did so in previous years (One guess: maybe more of these younger women breast-fed their children—a known protective factor).
Bioidentical HRT is still an option for women who have menopausal symptoms and want relief. Mainstream medicine doesn’t see any difference between BHRT and the kind of HRT that has been linked with breast cancer, but there is really a great difference. The progestin component of conventional HRT has been identified as the main problem when it comes to breast cancer risk. For example: women who took only estrogens (because they had had hysterectomies, and so did not need the uterine-protective effect of progestins) in the WHI study had no increases in breast cancer risk, and enjoyed bone and heart health benefits and protection against colon cancer.
I’m not suggesting you get a hysterectomy and start taking estrogen only! But natural progesterone, used as a transdermal cream, plus transdermal natural estrogen is a safe alternative to Premarin and progestins. Progesterone is not progestin, and never the twain shall meet. As always, I’ll recommend John Lee, M.D.’s books to those who are curious and want to learn more about bioidentical hormones.
